If you are travelling abroad you may need to be vaccinated, depending on the area you are travelling to and the activities planned. You need to allow at least eight weeks to organise these vaccinations, before you travel.

Download and complete the form below and return it to the practice. Our nurses will then get in touch with you to arrange an appointment to discuss the vaccinations recommended. This might involve two separate vaccination appointments and the final appointment will need to be at least two weeks before you travel, to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription which cost more than the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
